Kursus ini dirancang untuk memberi informasi tentang pentingnya dan nilai pembelajaran dari kecelakaan dan insiden, dan untuk memberikan pengetahuan dan keterampilan yang diperlukan untuk menyelidiki peristiwa tersebut dan berhasil mengidentifikasi dan mengatasi akar penyebab sehingga mengurangi risikolebih lanjut.

TUJUAN

Peserta diharapkan mampu melakukan investigasi kecelakaan.

  1. Pre Test
  2. Perform Accident Trend Analysis
  3. Identify top cost and severity contributors
  4. Identify top frequency causes
  5. Create reduction targets
  6. Examine Current Accident Investigation Process
  7. Are investigations conducted?
  8. Are corrective actions recommended?
  9. Identify areas for improvement or upgrade or establish the basis for initiation of accident investigations
  10. Determine Desired Outcomes
  11. Accident investigations determine how and why failures occur
  12. Use information gained to help prevent a similar or perhaps a more serious accident from occurring
  13. Identification of root cause
  14. Exposure of errors in the process that may have contributed to the accident
  15. Provide basis for elimination of identified hazards
  16. Decrease accident rates and workers\’ compensation costs
  17. Provide information for correction of unsafe acts or unsafe conditions
  18. Make clearly articulated recommendations for prevention of future accidents
  19. Prepare Training Program for Identified Investigators
  20. How to identify root causes
  21. What are at risk behaviors
  22. How to identify workplace deficiencies
  23. Identification of short-cuts or deviations from established procedures
  24. Rule and/or Policy enforcement
  25. Interviewing skills (who, what, where, when, why and how)
  26. Who was involved or contributed to the accident?
  27. Who witnessed the accident?
  28. What actions and/or conditions led up to the accident?
  29. Where did the accident happen?
  30. Why did the accident happen?
  31. When did the accident happen?
  32. Corrective Action Process
  33. Establish and explain the corrective action process
  34. Establish safety rules and policies and train the investigators
  35. Determine if you wish to have a return to work program and train your investigators in how the program works
